• Filling out an application form does not guarantee acceptance

• If you are accepted as a patient you will receive confirmation by email with a link to a registration form

• If you are not accepted you will receive an email informing you of the decision

• Being declined as a patient does not mean you cannot reapply at. a later stage

• You must fill out the below form for each member of your family you are applying for

• Our priority has always been to provide safe and timely care to our patients using all of the resources at our disposal. However, we must consider our current practice size and our capacity to provide high-quality medical care when accepting new patients.
