Medical Card / Doctor Visit Card
If have received a phone call or an email from our team accepting you as a patient AND you already have a Medical Card / Doctor Visit Card, you can apply to join our practice through the HSE website.
You must select Dr Gaile Cruz GMS 54757 as your doctor.
Dr Cannon’s list is full. If you apply under Dr Cannon, you will not be accepted. You will need to re-apply under Dr Cruz.
This will NOT affect which doctor you can see. When you attend the practice you can decide to see whichever doctor you wish.

Free GP Care for under8s commences registration from August 11th 2023
You must have a full valid Card before you are entitled to Free GP Care.
You must register with HSE for this as it is not automatic.

Private Patient List
- Our private Patient List is currently closed.
- We are NOT currently inviting applications.
- Notwithstanding that our private patient list is closed, we have accepted a small number of private patients, and will continue to do so where:
a. Family reunification: the prospective patient is an immediate family member of existing patients of the practice (e.g. parent / partner / child of a patient), or
b. Past patients: The prospective patient was a previous patient of the practice and had returned from a period away (e.g. abroad).
